Task Area 6 BPA: The objective of this task area is to provide management and staff for the NHLBI Center for Health Information (CHI). The NHLBI CHI maintains accurate, up-to-date information on a range of topics related to the mission and goals of the NHLBI, research conducted by the Institute, and health information on diseases of the heart, lung, blood, blood vessels, and sleep disorders. Integral to Task Area 6, JBS responds to public inquiries and develops and maintains standard responses for common inquiries that will be housed in the NHLBI CHI Inquiry and Inventory Management System; oversees and manages all warehouse and distribution; and ensures the NHLBI CHI maintains a system to archive and store historical NHLBI publications and materials. JBS also works with the NHLBI to develop and implement strategies for increasing the use of digital platforms for providing information from the NHLBI to the public while decreasing the volume of information provided as printed documents.

OHA seeks to obtain a pool of subject matter experts in 21 key subject areas, to extend OHA’s capacity and capability to offer TA to Coordinated Care Organizations (CCOs) across Oregon as well as assist other publicly funded health programs, tribal health programs, OHA contracted health systems, or other OHA program areas that work on health systems transformation. The State of Oregon, acting by and through its Oregon Health Authority (“Agency”), for the Public Health Division, Center for Prevention and Health Promotion, and Health Promotion and Chronic Disease Prevention Section (HPCDP) is seeking multiple contractors to design and conduct a variety of public health surveillance survey and data collection services. The requested services will include: developing survey and data collection plans; designing, conducting and facilitating data collection processes; meetings with contracting staff; convening stakeholder groups; coordinating survey and data collection review processes; designing data briefs, reports and presentations; and providing final data sets and products, including data processing documentation, descriptions of methods, findings and process improvement recommendations. HPCDP and the Public Health Division (PHD) overall are the recipient of these services.   1. Development of a strategic plan that is adaptable to changes in state government, the agency, or other reason that might require the plan to change; 
  2. Engagement and facilitation of meetings with leadership relating to strategic planning; 
  3. Mission statement, vision, goals, strategies and measurable objectives; 
  4. Organizational performance measures aligned with mission, vision, goals and objectives; 
  5. A work plan and schedule outlining the priorities for objectives, timelines and accountabilities for each goal; 
  6. Business structure, core functions, services provided and internal/external customers and stakeholders; 
  7. Financial and human resources needed to support core business functions and services;
  8. Facilitation of strategic planning methods, such as SWOT, SOAR or PESTLE, or something similar; or

Element #4: Change Management (CM)

Provide CM services to ensure the success of Authorized Purchaser’s business or strategic plan. Contractor shall be able to demonstrate its understanding and knowledge of CM and the benefits of CM to any business or strategic plan.
